New Patient Information

  • We are currently accepting new patients for pre-existing clients, and taking new clients on a case-by-case basis. We are currently not accepting second opinion cases at this time.

  • If you are interested in becoming a client at our practice, or have another pet you would like to add to your account, please fill out the form below and our new patient coordinator will be in touch as soon as possible.

  • Medical records must be sent before scheduling an appointment - records can be sent via email (animalhospitalofrockyhill@gmail.com), fax (860.257.1745), or by dropping them off in person during our normal business hours (Monday through Friday between 7:30-5:00pm, closed for lunch daily between 12:30-1:30pm). If the pet does not have any records, please let us know.
